By submitting this declaration, I acknowledge and agree to the following requirements and responsibilities as a volunteer at Carmichael College.

1. Volunteer Service and Blue Card Requirements

I understand that:
• If my volunteer service requires travel to an event or activity away from the College premises, I will arrange my own transport unless otherwise directed by my supervisor or the teacher or staff member responsible for the relevant activity.
• I am under no obligation to provide volunteer services to the College and may discontinue my volunteer service at any time by providing written notice to the College.
• I must not commence or continue volunteering at the College if I do not hold a current and valid Blue Card or exemption card, or if my Blue Card or exemption card is suspended, cancelled, withdrawn, expires or otherwise ceases to be valid.
• My Blue Card or exemption card must be linked to Carmichael College through Blue Card services before I commence volunteering.
• I am responsible for ensuring that my Blue Card or exemption card remains currrent for the duration of my volunteer service, including applying for renewal through Blue Card Services before it expires.
• I must immediately notify the College of any change to my Blue card or exemption card status, including if it is suspended, cancelled, withdrawn, expires or otherwise ceases to be valid.
• I must immediately notify the College of any charge, conviction, legal proceeding, investigation or other matter that may affect my eligibility to hold a Blue Card or exemption card or my ability to undertake volunteer activites at the College.
• I will comply with any requirements of Queensland Blue Card Services relating to my Blue Card or exemption card.

2. Child Protection Training

I confirm that I have viewed the ISQ Child Protection for Volunteers video and understand my responsibilities in relation to child protection and the safety and wellbeing of volunteering at Carmichael College.

3. Child Protection and Reporting Concerns

I understand that I have a responsibility to contribute to the safety and wellbeing of children and young people.
I agree that if I become aware of, observe, suspect or receive information regarding an allegation of harm, suspect harm, abuse or inappropriate behaviour involving a student, I will immediately report the concern to the Principal or an appropriate College staff member in accordance with the College's reporting Concern of Harm and Abuse Policy.
This includes concerns about harm that may have occurred in the past, is currently occurring or may occur in the future.
I understand that any obligation of confidentiality does not prevent me from reporting concerns about the safety or wellbeing of a student.
I understand that I must follow the College's Reporting Concern of Harm and Abuse Policy and any applicable legal reporting requirements.

4. Restricted Person Declaration - Qld Blue Card System

I confirm that:
• I do not hold a Negative Notice
• I do no hold a suspended Blue Card or exemption card
• I am not a disqualified person
• I have not been charged with a disqualifying offence for which proceedings have not been finalised; and
• I am not the subject of an adverse interstate Working with Children check decision that is currently in effect.

I understand that a restricted person is prohibited from undertaking restricted employment under Queensland's Blue Card Legislation, including certain child-related work.

I confirm that I am not a restricted person.

I understand that I must immediately notify the College if my circumstances change in a way that may result in me becoming a restricted person or otherwise affect my eligibility to undertake volunteer activities at the College.

5. Privacy and Confidentiality

I understand that, in the course of my volunteer service, I may have access to confidential information relating to students, families, staff, volunteers, College operations and the wider College community.

I agree not to disclose, share, reproduce or use confidential information except where authorised by the College or where disclosure is required or permitted by law.

I understand that my oblication to maintain confidentiality does not prevent or restrict me from reporting concerns about child safety, harm or abuse in accordance with the law or the College's Child Protection and Reporting Procedures.

I understand that a breach of confidentiality may result in the College discountinuing my volunteer service and may have other consequences where applicable.

6. Work Health and Safety

I agree to take reasonable care of my own health and safety and that of students, staff, volunteers and others who may be affected by my actions.

I agree to comply with reasonable instructions and applicable College policies and procedures relating to health, safety and emergency management.

I will promptly report any hazards, incidents, injuries or safety concerns that arise during my volunteer activities to the appropriate College staff member.

I understand that I must not undertake any activity outside the scope of my authorised volunteer role or for which I have not been appropriately instructed or supervised.

7. Code of Conduct

I agree to read, understand and comply with the Volunteers Handbook, Parent Code of Conduct and any other College policies and procedures relevant to my volunteer role.

I understand that my conduct as a volunteer must support the safety, wellbeing and dignity of all students and members of the College community.

I agree to conduct myself in a manner that reflects that values and expectations of Carmichael College and contributes positively to the College community.

8. Duty of Care

As a volunteer, I acknowledge and agree that:
• I may be required, under the direction or supervision of College staff and within the scope of my authorised volunteer role, to assist with the supervision and care of students.
• Where I am authorised and directed to undertake responsibilities involving and supervision or care of students, I will exercise reasonable care for their safety and wellbeing.
• I will follow reasonable directions provided by College staff and will promptly raise any concerns regarding a student's safety or wellbeing.
• I will only undertake activities that are within the scope of my authorised volunteer role and for which I have been appropriately instructed or supervised.
• I understand that the Civil Liability Act 2003 (Qld) provides certain protections from personal civil liability for volunteers undertaking community work in good faith, subject to the limitations and exclusions prescribed by the Act.
• I understand that these protections do not apply in all circumstances, including where I know or ought reasonably to know that I am acting outside the scope of activities authorised by the College or contrary to the College's instructions, and that I am required to comply with applicable laws, College policies and reasonable directions.

9. Volunteer Declaration

By submitting this application, I declare that:
• The information i have provided in my volunteer application and this declaration is true and accurate to the best of my knowledge.
• I have read, understood and agree to comply with the requirements and responsibilities outlined in this declaration.
• I will comply with the College's policies, proccedures and reasonable directions while undertaking volunteer activities.
• I understand that approval to volunteer is subject to the College's requirements and may be withdrawn if I fail to meet those requirements or if circumstances arise that affect my suitability or eligibility to volunteer.
• I understand that the College may verify my Blue Card or exemption card status and othe information relevant to my suitability to volunteer at the College.
• I will immediately notify the College of any change in circumstances that may affect my eligibility, suitability or ability to continue volunteering.
• I understand that if I no longer wish to volunteer at Carmichael College, I will notify the College in writing so that my volunteer service can be formally concluded.
