This form is to be used by all persons who desire to volunteer at the College.

In completing this volunteer registration form you will be providing Carmichael College with personal information. The College is committed to protecting the privacy and confidentiality of all applicants in accordance with the Australian Privacy Principles under the Privacy Act 1988 (Cth).
This information will only be used for legally required purposes.

Please note that all volunteer applications are subject to approval. Applicants will be notified once their application has been reviewed and approved. Approval to volunteer is valid for the academic year in which the application is completed and approved. A new application will be required for subsequent years.

All personal information will be stored securely and accessed only by those directly involved in the volunteer registration  process. If your registration is unsuccessful, your information will be retained for a limited period and then securely destroyed or de-identified, unless you consent to its retention for future purposes.

You may request access to your personal information held by the College and request corrections where necessary by contacting our Compliance Department.  

The College's full Standard Personal Information Collection Notice for Volunteers can be found on its Website. When you submit this form, it will not automatically collect your details like name and email address unless you provide it yourself.

Working with Children Positive Notice Blue Card

You are required to be in possession of a valid Working with Children Positive Notice Blue Card or an Exemption Card before commencing volunteering. You must not engage in volunteering within the College if you are not holding a current Working with Children Positive Notice Blue Card, Exemption Card or if your blue card lapses, is cancelled or withdrawn. All Working with Children Positive Notice Blue Cards and Exemption Cards will be linked to the College before your commencement as a volunteer.

14. I understand that:
- If volunteering service requires travel to event sites outside of the College premises, please arrange your own travel unless otherwise directed by your supervisor or teacher/staff in charge of your service area.
- I am under no obligation to provide any volunteer services to the College and may discontinue my volunteer activities at any time. 
- I must not engage in volunteering within the College if I am are not holding a current Working with Children Positive Notice Blue Card or Exemption Card.

16. Child Protection:
I agree that if I become aware of allegations of harm, inappropriate behaviour or suspected harm towards a student I will immediately report it to College staff and/or the Principal.  This includes incidents where I suspect significant harm has taken place in the past, is currently taking place or may take place in the future.

17. Restricted Person Declaration (Queensland Blue Card System)
I confirm that I have not received a Negative Notice, do not have a suspended blue card, am not a Disqualified Person, am not charged with a disqualifying offence, and have not received an adverse interstate Working with Children Check decision. I understand that restricted persons are legally prohibited from engaging in any child‑related work, paid or voluntary. I confirm that I am not a restricted person.

18. Privacy:
I agree that I will not, without prior permission, disclose any information regarding work practices, procedures, staff, students or the College community. In instances where it is known that someone has communicated information to unauthorised persons, the College may discontinue the services being provided.  Moreover, the Privacy Amendment (Private sector) Act 2000 also renders an individual liable for prosecution in the event of unauthorised disclosure of information. The legislated need for privacy and confidentiality does not negate or take precedence over the mandatory reporting requirements for significant harm set out in the Child Protection Act 1999 (Qld).

· take reasonable care to avoid any actions that could reasonably be foreseen as causing harm or injury to myself, students, or others.
· comply and co-operate with any reasonable instruction, policy and/or procedure relating to health or safety at the College.

21. Duty of Care:
As a volunteer at the College, I acknowledge and agree to the following:

  • I understand that teachers may delegate their duty of care to me under certain conditions (e.g., emergencies), and in such cases, I will owe the same level of care to students as a teacher.
  • I am aware that under the Civil Liability Act 2003 (Qld), I cannot be sued for acts or omissions done in good faith while volunteering, except where:
    • The act constitutes a criminal offence;
    • I am intoxicated; or
    • The act occurs outside the scope of my authorised volunteer activities.
